Payment Options and Information
Rehabilitative Health Services provides essential medical and mental health services regardless of the patient’s ability to pay. We offer many options in payment flexibility. Discounts and adjustment of fees are offered based upon household income and size for those who do not have any insurance. For those with adequate income yet no insurance, a cash price discounted fee is offered. For those with a limited income and no insurance, a sliding fee schedule is used to calculate the basic discount and is updated each year using the federal poverty guidelines. Please note that this is only offered on a limited, income-based, and case by case. Fee will be based and determined upon acceptance by the clinical director. All paper work must be filled out completely to determine eligibility for this program.
*Only counseling services, psychiatric medication services, and psychological testing services are eligible for the sliding scale fee discount.
For psychological testing services, patients who (a) do not meet guidelines for a sliding scale fee discount and (b) have out-of-pocket expenses due to insurance carrier denial of authorization may pay for services at the Medicaid reimbursement rate.
