What is CBRS?

A community-based rehabilitation service is a comprehensive approach to improving the quality of life of people with disabilities and their families. It involves working with community members to identify and address the barriers that prevent people with disabilities from fully participating in society. CBRS programs may provide a wide range of services, including education, employment training, transportation, housing, and health care.

In many cases, CBRS programs are run by local organizations or government agencies. However, they can also be implemented by international NGOs (Non-Governmental Organizations) or development organizations. Regardless of who runs them, CBRS aims to empower people with disabilities and their families so that they can lead full and productive lives.

What do CBRS Workers do?

CBRS  workers provide practical information and direct support to help people maintain their current skills, prevent symptoms from worsening, and practice newly acquired life skills to help maintain stability. CBRS teaches, promotes and supports increased independence in thinking and behavior. The goal of CBRS is to achieve the highest level of stability and independence possible for the individual.

Skills are taught to help individuals gain access to services, ensure that the member’s services continue and they maintain stability through coordinating with third parties. This includes assistance in maintaining psychiatric care, medication management, access to medical care, transportation and community supports.

Reforging Links to THE Community

Case Management

Case Managers assist clients with linking to services in the community to enhance the recovery process of a client navigating through a mental health diagnosis.

Case Management is a service offered to assist clients with a mental health diagnosis. Case Managers help with gathering information on resources, linking to and arranging for services in the community to assist in the recovery process.

What is Case Management?

Case management is a process that helps individuals with physical, mental, or developmental disabilities access the resources they need to live independently. Working with a case manager, clients can develop a plan that addresses their unique needs and goals. This plan may include services such as medical care, housing assistance, and job training.

A case manager also provides support and advocacy on behalf of the client and connects them to community resources. For many people with disabilities, case management is essential for leading a self-sufficient life.

What Do Case Managers Do?

Case managers provide a vital service to those in need, acting as advocates and coordinators to ensure that clients receive the best possible care. In many cases, case managers are the first point of contact for clients, and they play a key role in helping to connect individuals with the resources they need.

Case managers must be skilled in both assessment and communication, as they often need to gather information from multiple sources and then relay this information to clients, families, and other professionals. They must also be able to navigate complex systems, as they often need to coordinate care between different providers.

Who is it for?

Community-based rehabilitation services and case management are designed for a range of different individuals, from those experiencing early signs of mental illness to those who have more complex needs. The goal of community-based rehabilitation is to help each client learn the skills necessary to manage their condition on their own, with minimal support from service providers. This often involves individualized care plans that are tailored to the specific needs and strengths of each client. Case managers work closely with clients in order to help them navigate the healthcare system, identify resources in the community, develop coping strategies, and set goals for recovery.

Overall, community-based rehabilitation services and case management play an important role in helping people with mental illness gain skills and confidence that will help them thrive. Whether getting back into the workforce or reconnecting with old friends and family members, these services can help clients take back control of their lives.

What are the Benefits?

Community-based rehabilitation services help ensure that people with disabilities have access to the support they need to live independently. CBRS case managers work with individuals and their families to develop a care plan that meets the person’s unique needs. They also connect people with community resources, such as local disability organizations, housing assistance, and employment opportunities.

In addition, CBRS case managers provide ongoing support and monitoring to ensure that the individual’s needs are being met. Studies have shown that community-based rehabilitation services can significantly improve the quality of life for people with disabilities and their families. They also reduce the overall cost of care by preventing unnecessary hospitalizations and emergency room visits.
