We offer medical services for our clients as well as our community. Our medical clinic provides primary care for our clients which helps with an inter-disciplinary team approach. Community Based Rehabilitative Services (CBRS)
and Case Management (CM)
CBRS provides practical information and direct support to help people maintain their current skills, prevent symptoms from worsening, and practice newly acquired life skills to help maintain stability. CBRS teaches, promotes and supports increased independence in thinking and behavior. The goal of CBRS is to achieve the highest level of stability and independence possible for the individual. This service is for Medicaid recipients only. Accompanying CBRS, Case Management (CM) expands on and supporting CBRS services.
In these, skills are taught to help individuals gain access to services, ensure that the member’s services continue and they maintain stability through coordinating with third parties. This includes assistance in maintaining psychiatric care, medication management, access to medical care, transportation and community supports.
